In the state of Ohio, Massage Therapists are required to collect sales tax on all services rendered. Sales Tax in Hamilton County is currently 7.8%
In order to save money on your massages:
- If you plan on using a Health Savings Account (HSA) or Flexible Savings Account (FSA) credit card to pay for your massage, I will need to have a prescription written from a physician, chiropractor or dentist on file for you. Please make sure that the prescription lists any diagnosis codes so I can be sure to add that to your receipt. Please request a receipt as one will not automatically be given.
- If you would like to be exempt from paying sales tax and do not have a Health Savings Account (HSA) or and Flexible Savings Account (FSA) to pay for your massages, I will need to have a prescription written from a physician, chiropractor or dentist on file for you. The Prescription can state, “Recommended for massage therapy.” For intraoral work, please have your referring provider check "TMJ" and "CST/CFT" on the form.
Prescriptions are good for one year from the date that they are signed.
Feel free to download and print off this one-page referral form for your doctor to sign.
